2011 Clinical Breakout Session Goals
Conference planners encourage submission of abstracts for practical, tested, culturally appropriate strategies for:
- Assessing, treating and managing care involving patients with or at risk for chronic diseases in primary care settings
- Presentation should be innovative, address health disparities and have clear implications for primary care
- Other criteria include: practical clinical application, populations served, availability of outcome data, and relevance to both urban and rural safety net providers.

Abstract Review Process
All abstracts are reviewed by the Many Faces of Community Health Conference’s Clinical Content Advisory Group, a volunteer committee of professionals from primary care, health plans, public health, and community based organizations. Notification of proposal acceptance for presentation will be sent out by June 15, 2011. Accepted abstracts will likely be conferred a 90-minute oral presentation.
